Walgreens Expands GLP-1 Access With Prior Authorization, Pharmacist Support

Walgreens adds GLP-1 coverage navigation, Medicare Bridge support and pharmacist outreach to its virtual weight management service.

Walgreens announced Oct. 27 that it is expanding its virtual weight management service to help patients navigate commercial insurance coverage and prior authorizations for GLP-1 medications. The company is also adding support for Medicare GLP-1 Bridge participants and launching pharmacist outreach to every patient who begins filling a GLP-1 prescription.1

Through Walgreens Weight Management, patients connect online with a doctor or nurse practitioner and, if eligible, receive a prescription for an FDA-approved GLP-1 weight loss medication.1 Visits cost $49 with no subscription required, and the fee does not include medication cost. Patients already taking a GLP-1 for weight loss can also use the service's clinicians to support ongoing treatment, according to the company.

“Staying on a GLP-1 medication can be challenging for many patients. More than half of patients who begin GLP-1 therapy for weight management discontinue treatment within a year for a variety of reasons, highlighting the need for ongoing support, affordability solutions, and easier access to care,” said Rick Gates, chief pharmacy and external affairs officer at Walgreens, in a press release.1

The expansion puts Walgreens alongside CVS Health, which revamped its own GLP-1 weight management offerings in August, as both chains build out services aimed at keeping patients on therapy after the first fill.2

How Does Walgreens' Virtual Care Model Handle Coverage?

Walgreens said its weight management service can now help patients work through both self-pay and commercial insurance options, including checking prescription coverage and completing prior authorizations when needed.1 Patients eligible for the CMS Medicare GLP-1 Bridge program can also connect with virtual providers for help with eligibility, prior authorizations and setting up a treatment plan.

Care is delivered through independently owned Walgreens-affiliated medical practices using the company's telehealth platform. The service is available in every U.S. state except Alabama. For Medicare Bridge patients, it is also unavailable in Delaware, Kansas, Louisiana, Mississippi, New Mexico, Rhode Island, West Virginia and the District of Columbia.1

What Role Will Pharmacists Play After Prescription Fill?

After fill, Walgreens pharmacists will reach out to all patients who start filling a GLP-1 medication to answer common questions, share educational resources and connect them with additional Walgreens support.1 The company said the outreach is designed to help patients understand their therapy and stay engaged in their care.

“Walgreens pharmacists can support patients from the moment they begin exploring whether a GLP-1 medication may be right for them and throughout their treatment journey, helping them access information, resources and support along the way,” Gates said.1

Walgreens also launched an online GLP-1 resource hub, allowing patients to explore manufacturer savings card options, research information on the Medicare GLP-1 Bridge program and on products for nausea, dehydration and other gastrointestinal side effects.1

What Does this Signal for the GLP-1 Market?

Walgreens' move follows a similar effort by CVS Health in August, when it overhauled its direct-to-consumer weight management offerings.2 CVS cut the price of MinuteClinic online weight loss visits to $29, with no membership or recurring monthly fee, and said a pharmacist is available in person at 9,000 locations at no additional cost to help patients start and stay on therapy. It also announced a collaboration with Lilly that will let eligible Zepbound and Foundayo patients view transparent pricing, including reimbursed and cash-pay options, in the CVS Health app.

The two chains, which operate nearly 8,000 and about 9,000 locations respectively, are building comparable models around a flat-fee online prescriber visit and pharmacist engagement once therapy begins.1,2 Each also pairs that visit with help sorting through insurance, cash-pay and manufacturer savings options, targeting the affordability and access gaps Walgreens tied to high GLP-1 discontinuation rates.

Both chains are now extending their GLP-1 services past the first fill, toward the persistence gap Walgreens cited in announcing its expansion.
